We appreciate your interest in our therapy services. There are a few things that we need before services can begin. Requested forms must be completed and emailed prior to the first session or presented on the day of the session.
Prior to the Evaluation

If you plan to file with your insurance, a prescription from your child's primary care physician is required. The prescription must say OT Evaluation and Treatment for 6 months along with any ICD-10 diagnosis code(s). It needs to be dated within the last 6 months. We can help you get one if you do not have a current prescription.

You may be asked to fill out other necessary forms before the evaluation. This will ensure that we have accurate information to inform us of any evaluation materials we may need.
Evaluation
Post Evaluation

If we have not obtained it prior to the evaluation, please bring your child's insurance card.

Evaluations are 45-60 minutes long, depending on your child's age and tolerance.

During the evaluation, we obtain information through a variety of methods such as parent interview, skilled clinical observation and/or task analysis, guided play, standardized testing, and/or informal or standardized questionnaires.

At the end, there will be an opportunity to discuss what goals you and your child want to accomplish.

You will receive a PDF of the evaluation report 1-2 weeks after the evaluation.

In most cases, services cannot begin until the therapist receives their signed treatment plan back from the child's referring doctor.

Services will be scheduled within 1-2 weeks after the initial evaluation depending on when the treatment plan was received from the doctor or when/if prior authorizations are approved by insurance.

Based on child/family needs sessions are 30-60 minutes, 1-2 days per week.

The therapist may have activities or other suggestions to be completed at home for optimal carryover during the course of therapy.
